Consent to practice Yoga Therapy and Privacy policy
 

I consent to engage in Yoga therapy sessions and personal practice.

 

I understand that yoga therapy is a complementary therapy intended to work alongside other medical and allied health treatments. It is not intended to be a substitute for medical attention, examination, diagnosis, or treatment. I understand that, during the Yoga therapy sessions and home practice. I will complete practices designed for my condition as agreed with my Yoga therapist.

 

As in the case of any physical activity, the risk of injury cannot be eliminated. I understand that I am the best judge of evaluating my body and my child's pain and physical limitations. Should any symptom, pain, discomfort, or any other concerns change, or new ones occur at any point, I will inform the Yoga therapist as adjustments may be necessary. I am aware that my yoga therapist is under supervision training and understand she may consult with Senior Therapist or health professionals about my progress to help improve my treatment, but without disclosing my identity. I understand that by the 13 Privacy principals (APPS) of the privacy act (1988) any personal information is store in a manner that reasonably protects it from misuse or loss and from unauthorised access, modification, or disclosure. This information may only be disclosed where required or authorised by law to third parties (such as other health professionals) where I consent to this disclosure or to myself upon written request.
